Aim of the webinar: 

The webinar will provide an overview of avoidable transfusions reported to Serious Hazards of Transfusion (SHOT), the UK haemovigilance scheme which has been in operation since 1996. Recurring themes from reports analysed will be discussed, useful resources signposted, and key safety messages will be highlighted. Strategies to address gaps in practices and enhance patient safety will be explored. While the discussions be focused on the data from UK, the key principles of effective patient blood management and improving practices to ensure optimal use of blood is relevant to all transfusion staff from around the world.

Vera has worked as a Biomedical Scientist since 2011, with experience in haematology and blood transfusion including blood donation services and patient testing. In 2018, she joined the Red Cell Immunohaematology department at the National Blood Service, specialising in serology and flow cytometry.

In 2023, Vera became part of the Serious Hazards of Transfusion (SHOT) team, where she is responsible for reviewing events related to anti‑D immunoglobulin errors, immune anti‑D, wrong blood in tube, and avoidable, delayed, or under/over transfusions. She also co‑authors the corresponding chapters of the Annual SHOT Report, supporting improvements in transfusion safety through shared learning.’
